CONTACT LENS HISTORY

Determine if the patient has worn contact lenses before.

If yes:

   When?
   For how long?
   What type? (rigid or soft, spherical 
      or toric, daily or extended wear)
   Why did they discontinue?
   What problems did they have?

It is important that you do not make 
the same mistake(s) that the previous 
fitter did.
